Tourist visa β€” document checklist
  • Schengen visa application form, completed and signed by the applicant
  • Passport issued within the last 10 years, valid at least 90 days beyond the visa's expiry, with at least two blank pages, plus photocopies of the first and last pages
  • Two recent (under six months) colour photographs on a white background, face covering about 80% of the frame, to ICAO specification
  • Travel medical insurance covering at least EUR 30,000 for all Schengen countries, valid for the entire trip including the arrival and departure dates
  • Flight tickets β€” a booking is sufficient, as there is no need to buy the ticket until the visa is granted; if visiting several Schengen states, add intra-Schengen transport bookings
  • Proof of accommodation: hotel or travel-agency voucher for the Slovakia stay with confirmation of payment, showing the agency's name and address, voucher number, guest names, arrival and departure dates and the services booked
  • Cover letter signed by the applicant explaining the purpose of the visit to Slovakia and the full travel itinerary
  • Proof of funds: original personal bank statement for the last three months, stamped and signed by the bank, plus Indian income-tax return acknowledgements for the last two assessment years
  • If employed: salary slips for the last three months, employment contract and the employer's leave-approval letter; if self-employed: the company's certificate of registration
  • For minor applicants: birth certificate, copies of both parents' passports or ID cards, and notarised consent from any parent not travelling

Business visa β€” document checklist
  • Completed and signed Schengen visa application form.
  • Passport issued within the last 10 years, valid at least 3 months beyond your return, with at least two blank pages, plus a photocopy of the first and last pages.
  • Two recent (under 6 months) colour photos on a white background, to ICAO / Schengen biometric specification.
  • Cover letter from you setting out the purpose of the visit and the full travel itinerary.
  • Invitation letter from the Slovak organization or business partner, stating purpose, duration and itinerary for the whole Schengen stay (original hard copy).
  • Official dispatch / employer letter confirming your current occupation and the reason for the trip to Slovakia (original hard copy).
  • First-time Schengen visitors: the Official Invitation Form verified by the Slovak foreign (border) police department (original hard copy).
  • Flight tickets and proof of accommodation (hotel bookings) for the whole stay.
  • Travel medical insurance valid for all Schengen countries, covering at least EUR 30,000 for the entire trip.
  • Proof of funds: original personal bank statement for the last 3 months (bank-stamped and signed) and Indian Income Tax Return acknowledgments for the last two assessment years.
  • If employed: last 3 months' pay slips, employment contract and employer's leave approval. If self-employed or a company owner: certificate of company registration.
