Document checklists
Tourist visa — document checklist
- Schengen visa application form, completely filled and signed by the applicant
- Passport issued within the last 10 years, valid at least 3 months beyond your scheduled return, with at least two blank pages, plus a photocopy of the first and last pages and of any previous Schengen visas
- Two recent colour photographs (not older than 6 months), white background, ICAO specification, with the face covering at least 80% of the frame
- Cover letter signed by the applicant stating the purpose of the visit to the Czech Republic and the full travel itinerary
- Flight tickets; intra-Schengen flight, train or car-rental itinerary if visiting several Schengen states
- Confirmed hotel or accommodation bookings for the whole stay; if staying with family or a friend, the host's proof of accommodation or sponsorship
- Travel medical insurance valid across the Schengen area with a minimum of EUR 30,000 cover for the entire trip
- Original personal bank statement showing movements over the last 3 months, stamped and signed by the bank
- Indian income tax return (ITR) acknowledgments for the last two assessment years
- If employed: last 3 months' pay slips, employment contract and employer's leave approval; if self-employed or a company owner: certificate of company registration
- For tourism: travel-agency confirmation of an organised trip or other document indicating the travel plan; marriage certificate if married and not shown in the passport; copy of the ration card, if applicable
Business visa — document checklist
- Completed and signed Schengen (Type C) application form
- Indian passport valid at least 3 months beyond departure from the Schengen area, issued within the last 10 years, with at least 2 blank pages, plus copies of previous visas
- Two recent biometric passport photographs to Schengen specification
- Invitation letter from the inviting Czech company or organisation stating the purpose and duration of the visit and who bears the costs
- Cover letter from the applicant explaining the purpose of travel and the full itinerary
- Employer's letter or no-objection certificate confirming your position, salary and approved leave
- Proof of business or employment: employment contract and last 3 months' payslips; for business owners, company registration and GST returns
- Personal bank statements for the last 3 months and income-tax returns for the last 2 years
- Flight tickets and day-by-day travel itinerary
- Proof of accommodation, such as hotel bookings covering the entire stay
- Travel medical insurance covering all Schengen states for a minimum of EUR 30,000 (Czech missions accept only insurers on their approved list)
- Proof of legal residence in India if you are not an Indian national
